Registration is $300 Per play

 We accept partial payments with the full 300 do by July 1st if you sign your child up but do not pay in full by July 1st you'll be charged the $25 registration fee and a refund will be processed for the remaining balance.

 Fall sign up start March with a July 1st deadline in order to ensure we are able to obtain custom, personalized jerseys there is a 6-8 week turn around time. IF YOU REGISTER AFTER JULY 1ST YOUR CHILD JERSEY WILL NOT BE PERSONALIZED.

 Each player must have it up-to-date sports physical NOT a regular school physical. This is IHSA Sports physical that can be obtained from their PRIMARY DOCTOR or QUICK CARE and some FIRST CARE locations. IF THE PHYSICAL IS OUTDATED THE PLAYER CAN'T PLAY OR PRACTICE UNTIL IT UPDATED PHYSICAL IS OBTAINED

 EACH PLAYER MUST HAVE A OFFICIAL STATE ID. THESE CAN BE A OBTAINED THROUGH THE DMV OFFICE . YOU NEED TO TAKE THE CHILD'S BIRTH CERTIFICATE THE SOCIAL SECURITY CARD AND A PARENT/GUARDIAN WITH AN ID WITH THE CORRECT ADDRESS. THE FEE IS $5.00per ID. THE LEAGUE USES THESE TO VERIFY PLAYER IDENTIFICATION. IF YOUR CHILD DOES NOT HAVE A STATE ID THEY CAN'T PLAY OR PARTICIPATE IN THE GAMES

 

WE MUST HAVE COPY OF EACH ATHLETE'S BIRTH CERTIFICATE

 EACH PARENT AND ATHLETE MUST SIGN AND INITIAL, DATE THE PARENT AND ATHLETE CODE OF CONDUCT FORMS PRIOR TO PLAYER PARTICIPATION.

NO EQUIPMENT BE WE HANDED OUT UNTIL PAID IN FULL! Keep in mind at some point we will be practicing with equipment, so we encourage to pay in a timely matter. Payment arrangements are an option, half now and half later
